removing inquiries
There is a sample letter in this website that I use....You can either mail it to the creditor, or try to save a stamp and fax it. Sometimes it is hard to find the fax number......you will get about 50% of them removed with persistence.

Re: Unauthorized Credit Inquiry
Dear American Express,
I recently received a copy of my Experian credit report. The credit
report showed a credit inquiry by your company that I do not recall
authorizing. I understand that you shouldn't be allowed to put an
inquiry on my file unless I have authorized it. Please have this
inquiry removed from my credit file because it is making it very
difficult for me to acquire credit.
I have sent this letter certified mail because I need your prompt
response to this issue. Please be so kind as to forward me
documentation that you have had the unauthorized inquiry removed.
If you find that I am remiss, and you did have my authorization to
inquire into my credit report, then please send me proof of this.
Thanking you in advance,
Jane Caveat-Debtor

I ran accross this letter a while back and it's a bit more forceful when dealing with a CA. Not sure how well it would work with Amex though:
is it required to dispute with the credit reporting agencies, or

The Fair Credit Reporting Act says that credit grantors with a permissible purpose may receive your credit information from credit reporting agencies without obtaining your consent. If in the recent past you have visited or had any dealings with any of the businesses listed in this section, they may have had a permissible purpose to review your credit. This listing is simply a record of those who have reviewed your information.
If you have evidence that someone reviewed your credit information for a fraudulent purpose, please explain the circumstances in writing to us and we will investigate your claim. Write to us at Experian, PO Box 9556, Allen, TX 75013. Be sure to include your full name, including middle initial and generation (such as JR, SR, III, etc.), current mailing address, Social Security number, date of birth, and your previous addresses for the past two years. Please send us one copy of a government issued identification card, such as a driver's license, state ID card, military ID card, etc., and one copy of a utility bill, bank or insurance statement, etc. Make sure that each copy is legible (enlarge if necessary), displays your name and current mailing address, and the date of issue (statement dates must be recent). We are unable to accept credit card statements, voided checks, lease agreements, magazine subscriptions or postal service forwarding orders as proof. To protect your personal identification information, Experian does not return correspondence sent to us.
