Vacate a Judgment

I was able to vacate a Judgment this week since I was never served summon. I was also able to settle the debt with the creditor before I went to see a Judge this week. The Judge verbally agreed that she is vacating a Judgment since I was never summoned and the settlement is already taken place. Now the Judge asked me write a letter to her requesting to vacant a Judgment and then she will sign off on it.
Can some tell me where can I find this kind of letter online because I would need to send a copy of this letter to all the 3 credit beurues. Please Advice
