Letter requesting PIF and one suggesting a payment plan

Does anyone have copies i can use? I'm looking for one that says I've paid you too much already (ie 600 on a 300 loan) and I'm not paying anymore please mark my acct PIF AND one that suggests a payment plan for those companies that I still owe principal too.
I'm sorry I'm just too frazzled and stressed to figure out my own !
