I would like to send a letter requesting not to contact me at work or any third party on behalf. Do you know the address I can mail/fax this request?

Email them via the customer service address on their website. They shouldn't be calling you at work or calling third parties if you're keeping in touch with them. Are you in contact with them?

Yes, I am in touch with them and I have asked them not to contact me at work. But I am not able to accept the payment arrangements they are offering at this time (they are too high) and told them I would make payments as best I could in amounts that would allow me to still take care of my family and I was told they would continue to call.

There's not much you can do about that. They're the original creditor, not a third party so FDCPA doesn't apply to them.

I suggest you schedule calls with them. If they need them to be daily, then set up a time each day that is convenient for them to contact you at the number of your choice.
