Withdrawal of electronic consent letter needed

Does anyone have a basic letter to send to pdl to withdraw electronic consent for them to take $ out of checking account?

Here is the link to Polly's pdl settlement letter, at the beginning it revokes ACH authorization, if you don't want to use the entire settlement letter/inserting your state's pdl laws, you can just use the beginning withdrawing your ACH authorization.
