I posted a question a few weeks ago why Innovis felt they were exempt from the fact act. My query was based on the fact that their website specifies that residents of certain states are entitled to a free copy of their report, but made no mention of the rest of us, or even of the FACT act as a whole.

So, I sent a letter off (below) and in a little over a week they sent my report to me with out any question. If you want your Innovis report, feel free to use my text below as a starting point for your request. Identifying information has been changed to protect my privacy and comply with forum rules. Also I Do not have a phone number and use my cell phone. In my original letter I stated this fact and gave my cell number, but also instructed that that number is not to be included in their files.The identifying information I provided was what they stated on their website would be required in order for them to process the request, though for a $10.00 charge.

To Whom it May Concern

I am writing today to request any credit report file which your organization may have for me. My required information is as follows:
James Doe 7625 Quiet Lane, Small Town, IL 11111. I have resided at this address for four years. My Social Security number is XXX XX XXXX. I was born on July 31, 1972. I am currently employed by Acme Widgets Inc. My home phone number is (555)555-5555. I am enclosing a copy of my State Identification card. It is my policy not provide my drivers license as identification.

It is my belief this is a request which is covered under the FACT ACT, therefor this report should be provided at no cost. If this is not the case, please respond with a reason why this request cannot be honored under the FACT ACT.
