Credit Card Debt

I am a single mother and I work full-time at a hospital however I am a little over$6,000 in credit card debt. My minimun payments are killing me on 6 cards. I have a $211.00 car payment every month on a 1998 Nissan Altima and its worth about $1,400 but I owe a little under $3,000. I cant afford the minimum payments anymore because I dont make enough money. My credit score last time I checked was 569. I dont know what to do I am starting to have the creditors calling me again. I need help.
