First thing I would like to say is congrats to you for tackling this problem at your young age. I am proud of you, this shows that you are certainly mature for your age, good first step.
Do you work? Do you need the credit card? If not call capital one and close the account. Continue to make the payments and don't be late. This will improve your score.
You may be eligible for student aid to go to school. Where do you live? Where there is a will there is a way! What are interested in going to school for? All these questions have purpose. Some occupations have apprenticeship programs, etc., there are also corporation grants that will assist you if it is in their area of intrest. I will try to help you out with the school funding, tell us a little more about your situation though so we have more to work with.
Don't worry about the transportation till you know where you will be going to school, you may be in walking distance.
For the medical bills there should be a program in your state that helps with them. If not, medical bills will often accept very low payments.
I will look for your response, relax, don't be so stressed at your young age.
