ACH Revocation letter for the Bank?

Do I need a formal letter to the bank if I am trying to stop an ACH transaction from going through? And if so does anybody know where to find one? I've seen the letter for the actual PDL's but haven't been able to find one directed at the bank. I know there is a 3 day period before the transaction goes through that an ACH must be withdrawn per Federal Guidelines. I guess my worry here is that I go to the bank and talk with them, they say we'll stop it, then it still goes through. I would like to have a paper trail that shows date and time that I revoked ACH rights.
I know the response will probably be that I should have closed the account. I will, but I still have a paycheck going into that account and it's too late to divert it to a new account. I need that account open in order to receive the paycheck in a timely fashion. I hope this post isn't too confusing!

Actually, all you need to do is send a copy of this letter to the lenders and take a copy to the bank, to show them you have officially revoked any further debits on your account from these lenders.
