have big problems trying to prepare for debt relief help - they want addresses i can't find & i'm not sure how the letter to stop debiting my account should be worded

You will only find the names of the creditors and the collection agencies to whom you owe the debt in your report and not their address. To find out their address, you need to check the BBB website.
