letter to employer HR dept

I have closed my bank account and opened a new one, my bank was very helpful. Now I need advise on what to do in regards to notifying my HR dept. I keep reading about a letter that I should give them.. What is it?..and is there an example or form letter someone can share, secondly what does this letter prevent the PDL's companies from doing

so i'm clear on this voluntary wage assignments letter.

(Name of my HR/Payroll),

This is a request to revoke any voluntary wage assignments that I may have allowed on account of (name of the PDL). Please have this processed at the earliest.

please advise if I will need to submit a letter stating such for each payday loan, or can they all be listed in the one form letter. Secondly, what is my HR Manager to do with the letter..is she to forward it on to our cooperate office? and if so what do they do with it. does this stop any garnishments. I read to keep some of my pride I can explain I have been scammed by a fraudlent company and thus the reason for the letter.

Does this letter over ride an formal garnishment being served by the sheriff

Have you read the link in my signature line on how to deal with illegal loans?? You send a letter to your PDL stating....

Dear Name of PDL...
I hereby revoke any and all voluntary wage assignments I may have signed with your company affective immediately.


After you send a copy to the PDL, make a copy and forward it to your payroll department. Writing your payroll department is useless...you just need to so them you revoked it with your payday loan.

You would only receive a visit from a sherriff if they had already sued you and gotten a judgment.
