It is revocation of wage assingment. When you sign for a pay day loan, you agree to voluntary wage assignment if you default on the loan.
To revoke it:
"I hereby revoke any and all voluntary wage assingments I may have signed in regards to account #_____________ effective immediately."
After you send it to the PDL, give a copy to your payroll department.
