-I did not authorize the party listed above to debit my account
-I authorized the merchant to originate one or more ACH debit entry(ies) to my account, however on______(mm/dd/yyyy) I notified the merchant that I revoked the authorization. This requires me to provide a copy of the merchant's cancellation policy and proof of written cancellation or a cancellation number
-My account was debited before the date I authorized. The date I authorized was _______
-My account was debited for a different amount than I authorized. The amount I authorized was $_______
-Other (must specify)
Trying to figure out which one applies and which one won't have the $250 temporary refund returned.
Did you do an ACH revocation

Yes I did. After revoking it

I checked off the first one
I checked off the first one and sent it in. Hoping that it isn't reversed.
Update....
So I got a letter from my bank today that the temporary credit for the unauthorized transaction is now a permanent credit and the investigation is closed. Haven't heard from QXL Online about this.
