Just send it with your name and the last 4 digits of your social. They won't honor it anyway. It's more important that you go through the motions in case you need to take further action. You don't need to make their lives any easier.

i might be confused here, but my take on this is that the poster
i might be confused here, but my take on this is that the poster is asking what to put on her validation letter since she does not have her account number.
what i have done in that situation was put my whole name, address, phone number and put "Re. Any accounts that you may have with me."
i did not give them any other information, since they knew who i was and i was not up for playing their games. of course, do not send payments unless they validate (which they probably will not). i would not even bother calling them for the account number, you do not really need it, i would just send it certified return receipt and move on.
