I don't see where it would hurt to send along with your other information.. but before you send I still think you want to have your bank account more secure - make sure you give copies of the ACH authorization revocations to your bank. that will help protect you if they try to debit after the fact. If the bank allows them to go through after they have been notified that you are revoking ACH authorizations then the bank becomes liable to repay you.. those amounts.
