- ACH Revoke letter question

Confused here on ACH template letter. I have loans out from store-front PDL doing business as CSO.
Loan doc says lender is xxx. Do I send the letter to the PDL like Rent a center or to the lender? Quick.... please help.
Also on template, acct number is my bank acct number, correct?
