Need Some Advice on Writing Letter

Hey Guys:
I have been working with T&C for the last three months and they have been paying my bills respectfully.
However, I am curious on what my current balances are with these companies. I want to write them a letter requesting that they send my balance to me and also to see whether they will finally eliminate my debt from them completely and/or work out a settlement. However, I am stumped as to start the letter. Can someone help me?
And if it wouldn't be a problem, does anyone have phone numbers and/or fax numbers for the following:
Ten Dollar Payday Loan
Cashnetusa
Cash Central
United Cash Loans
One Step Cash.
