want to consolidate loans

I have five small loans that I would like to consolidate all into one payment, if it is possible I would like to have the all paid off, then make one monthly payment. In the past I went from making great money to almost $700 a week to only making about $550 every two weeks. I now have a good job again, but i had gotten behind on the payments and now im in a whole and my credit is starting to go bad, I have two credit cards, and two other bank loans :( think you for taking the time to veiw my request and look forward to hearing back from you
