Credit Card Settlement Offer Letter

Okay, I know I saw this somewhere, but now I can't find it. I have some really crappy credit cards that I am getting nowhere with when it comes to paying them off -- annual fees, monthly fees, payment fees, etc. -- I remember seeing a letter template that tells these company that I am having a hard time paying them and that I'd like to close the account with a lower apr, etc.

Does anyone know where that link is?
