Bankruptcy & continued wage garnishment

We filed for bankruptcy and submitted documents to my employer and the collection agency that holds a wage garnishment for me - they will not dismiss the garnishment and my employer continues to hold the money. I want to file a motion of contempt with the court but not sure how to word it - can anyone help? (This is for medical debt)

Just verifying: Are you talking about garnishment, as in the one which is court ordered or are you talking about voluntary wage assignment?
