Way over my head - no way out

I am in WAY over my head. I don’t know how it happens. I’ve filed bankruptcy before, lost a home to foreclosure and am now in the same spot again! I am a person who obviously cannot handle finances. I have a checking account that I’ve had since 1988. The only reason they haven’t closed the account is because they make too much money off of me in overdraft/NSF return fees. If I had back all the money I’ve paid in those I’d be rich!
I don’t know what to do at this point. I have 3 small credit cards ($300 - $750 limits) which are all over limit and past due. I have 3 payday loans out ($400, $500 & $600) and an installment loan ($1,500). I owe two relatives (one $2,135 & the other $2,500). I have an heirloom wedding ring in a pawn shop ($1,370). Total with fees/interest it is totaling close to $12,000. I have tried everything I can to obtain a loan to cover these and close all accounts (including the checking account) but, of course, with my horrible credit I cannot obtain it. I have a good job, steady income and can afford to make a $500 monthly payment. I’ve posted an ad on Craigslist hoping someone would be willing to loan me this amount over 3 years at 20% interest with signed legal documents and a direct wage allotment to an account of their choice. Of course, not one legitimate response was given.
I do not want to continue in this vein. I want to pay off all I owe, close all the accounts that get me into trouble and live on a cash-only basis.
I don’t know what to do. I’m sorry I keep repeating myself, but I’m scared I’ll lose everything (which to me at this point in my life is not monetary). My significant other does not know of these issues and if he did he would leave me. He has perfect credit and has helped me through this the last time. He is one of the relatives I owe ($2,135) which is furniture in his name that is coming due August 26 that I promised to pay. My daughter is renting a house from him (which he is taking a loss on) and if he found out and kicked me out it would not only affect me but her and her family as well. I would also have nowhere to go as I could not possibly get into a place of my own with my credit as it is. He would not understand as he has never had a late payment in his life and has never had to resort to payday loans as he can get any loan he would require. He hates them with a passion and is very verbal about it.

Personally I'm not very fond of with the idea of taking further loan to clear of the past, since things may soon go astray especially with poor money management skills.
There could be couple of ways to settle the debts that you have- debt consolidation and debt settlement, but you need to ascertain which one would be suitable for you.
