Need help with first step on getting out of debt nightmare

My husband and I have made a lot of bad choices and kept getting ourselves further into debt, well it has finally caught up with us over the last 2 yrs, the debt is a combination of credit cards and home equity loans, this is really scary because we are barely treading water. Does anyone have any advice on how to get started. I've been reading some self help books on getting out of debt they all have some good ideas on not spending and cutting cost where ever you can, but the funny reality is there is no extra money to spend we have more monthly bills than money. :shock:
