Where do I get contact information for a payday loan?

I have a payday loan and I cannot find the contact information. The website address I saved to contact them was wrong. The debit in my checking register is "E-PAYDAY FEE DEPOSIT." Can anyone help me find the contact information so I can resolve this?

The website I applied to never sent me a confermation so I thought I didnt get the loan and didnt save anything. Then all of a sudden debits were coming out of my account. What type of information can I get from my bank?
