Change of Venue for Suit in Small Claims Court

I am being sued by SOS Money Call ( a loan like Cash Call) in small claims court and I want to request a change of venue. Does anyone know what needs to be in the letter to the court to make this request? I want to be accurate in my request. Thanks.
