unable to locate this PDL-QXL

Hello does anyone have a physical address for QXLonline.com or a fax number? The 1866-214-1694 is no help it just rings. Then voicemail comes on for you to leave a message but in the mean time (like yesterday) I receievd 5 calls back to back on my work phone then on my cell 6 times. I want to get this letter to them but I REALLY need the contact info
