Notice of Defense from PDL

My employer received a Wage Assignment notice that states we must provide a Notice of Defense within 5 days of the Notice to garnish wages. What does that mean?? How can I find that form??

Sounds like garbage to me. Just send the payday loan company an email or letter revoking their right to any wage assignments you may have signed, tell them they no longer have your permission to use this wage assignment, date it, and copy it, give the copy to your employer. Done. Don't worry about any request for a notice of defense, that's bull crap, the law says you can revoke it at any time, period.
