A cease and desist letter doesn't have to be fancy..And there really isn't any one specific letter, although I think there is a copy of a successful one floating around here.
All you need to do is send a letter with your name, date, and account number. Tell them that you do not authorize them to contact you by phone, either at home or at work. Any correspondence needs to be by email or postal mail. Make sure you send it certified, and have the return receipt sent back to you!

Seriously, it worked for me. If they call you, they have violated the order. You should be ok, it will definitely trickle down.

So I took what you've written and poked around at some links and from the looks of it, it seems as those cease and desist letters are only for third party collectors? Can I not send one to my actual credit card company, then?
