unsecured loan for credit card or Debt consolidation program

I am very upset about my credit card debt of 15000,i do not know which is good to free from debt ,either applied for unsecured loan for credit card debt or debt consolidation program.I did not heard any success story about debt consolidation program only heared horrible situation.
Is there is any financial company in california which can give loan in my case because my credit score is 580 which is poor and repute bank in the california refuse to give loan to me .
Any help or suggestion in my matter will be appreciated
