Can disputing old items such as collections really hurt yoru score? I've heard about 'report dates'...what impact does this have?

You really can't dispute a valid entry even when it's old. But disputing wrong items in the credit report can actually affect your score positively. You can dispute the following

-Debts that don't belong to you

-Records that are staying longer on the report than their periods

-Status of the accounts, such as- correct status of the closed, settled accounts.

-Wrong name, address and SSN, etc

Hope it helps.
