What documents do I need to have for filing income tax returns?

I feel like such a failure. I took out one hiil(high interest installment loan), which led to another and another. These are worse than payday loans as they do not offer extended payment plans. I’m sick with worry and ashamed of myself. I signed up with a debt settlement company and they took 4 months of my money and disappeared. Now I’m behind and one is already in collections. What can I do? I live in Kansas and earn good money. This is getting so out of control and now my credit has been affected. Please, can anyone offer any advice?

From the perspective of credit report and score, which is better settlement or bankruptcy?

After getting discharge from my debts, can I still owe something on my debt? Can a creditor call me for such a debt?

I have 2 credit cards and somewhat I can manage them properly. I want to take out a new credit card as I'm geetting good offers. My question is that how much my credit score can drop if I shop for a new credit card? Is there any way to keep the score intact?
